Special Needs

Sumter County Emergency Management is required by Florida State Statute to maintain a registry of people that would require additional assistance during an emergency or disaster.

This registry is operated and maintained by the Florida Department of Health (FDOH) in Sumter County. People can registry in two ways:

  • Print out a SPECIAL NEEDS REGISTRY FORM and mail it in to ATTN: Special Needs Registry Coordinator , P.O. Box 98, Bushnell, FL 33513 and address it to "Emergency Management"
  • Go online and register here

FDOH in Sumter County will then review your application and inform you of your status. If you meet the criteria, you will be able to evacuate to the Special Needs Shelter at the Wildwood Community Center located at 6500 Powell Road, Wildwood, FL 34785.

FDOH in Sumter County reviews your application and you exceed the criteria, we will identify a temporary healthcare facility for you to evacuate to.

Do not wait until the last minute to register. Sumter County reserves the right to cease application processing during an emergency or disaster.

When there is an impending emergency or disaster that requires you to evacuate, FDOH in Sumter County will notify you and ask if you would like to evacuate. A transportation representative will then pick you up and transport you to a shelter facility.
